2. Report of Birth of the child duly registered at the Embassy (click here).

3. Valid passports of the parent/s with a photocopy of the passport’s data page.

4. Personal appearance of the applicant and Filipino parent/s at the Embassy.

5. Payment of US$60.00 for the passport processing fee/(visit this page for the LAK equivalent). Payable in cash (USD or LAK) and BCEL OnePay (LAK).

REMINDERS:

• Other relevant documents may be required by the Consular Officer, depending on the circumstances of the passport applicant.
• The processing period is approximately six (6) to eight (8) weeks.
