A. Documentary Requirements

1. Personal appearance. The affidavit needs to be signed before an authorized officer of the Embassy before it can be notarize
2. Original passport of the affiant of the affidavit
3. Other supporting documents that form part of the document for notarization via Acknowledgement 

Other relevant documents may be required by the Consular Officer, depending on the circumstances of the applicant requesting for the consular services.

B. Procedures:

1. Affidavit must be prepared by the affiant.
2. The affiant ensures that all the information in his/her affidavit is correct and the complete names (first name, middle name, last name) are reflected in the affidavit
3. Pay the processing fee of USD 25.00/(visit this page for the LAK equivalent). Payable in cash (USD or LAK) and BCEL OnePay (LAK).
4. Processing period is one (1) working day.
